PLEASE READ BEFORE MAKING YOUR APPOINTMENT
Surgical masks are required to enter all Eisner Health centers. Cloth masks are not acceptable. If you or your child do not have a surgical mask, please request one at the front desk.
If you have been diagnosed with COVID-19 or have a pending test, please call to change your visit to a virtual visit.
Stay up to date on all immunizations including your COVID-19 vaccine and boosters. All vaccines are available at Eisner Health for you and your family.
Additionally, we recommend that you wash your hands frequently and for at least 20 seconds. Lather and wash both side of your hands, between your fingers and under your nails.
For your convenience, hand sanitizer is available at every site, and we recommend you use it frequently.
Eisner Health offers two vaccines against COVID-19: Pfizer and Moderna.
The Pfizer vaccine requires two doses and is approved for anyone 5 years of age or older.
The Moderna vaccine requires two doses and is approved for anyone 18 years of age or older.
A second booster dose of the Pfizer-BioNTech COVID-19 Vaccine or Moderna COVID-19 Vaccine may be administered to individuals 50 years of age and older at least 4 months after receipt of a first booster dose of any authorized or approved COVID-19 vaccine.
A second booster dose of the Pfizer-BioNTech COVID-19 Vaccine may be administered to individuals 12 years of age and older with certain kinds of immunocompromise at least 4 months after receipt of a first booster dose of any authorized or approved COVID-19 vaccine. These are people who have undergone solid organ transplantation, or who are living with conditions that are considered to have an equivalent level of immunocompromised.
A second booster dose of the Moderna COVID-19 Vaccine may be administered at least 4 months after the first booster dose of any authorized or approved COVID-19 vaccine to individuals 18 years of age and older with the same certain kinds of immunocompromise.

Recommendable for immunocompromised persons: Pfizer and Moderna require 28 days after the second dose.
Johnson & Johnson:
The Johnson & Johnson vaccine is one shot and is approved for anyone 18 years of age or older. If you received the Johnson & Johnson COVID-19 vaccine, we recommend receiving a Pfizer or Moderna booster. We do not currently offer the J&J vaccine but can refer you to an organization that offers this type of vaccine if needed.
