Community Health Worker

The Community Health Worker (CHW) will work closely with medical providers, primary care teams, and social services agencies to provide outreach and support to patients enrolled in the Health Homes Program (HHP) to improve their health and general well-being. The CHW will work in both clinical and community-based settings, including patients’ homes. The CHW will serve as a bridge between the patients and the social services system. A demonstrated commitment to working with families living in disadvantaged communities, of diverse racial and ethnic backgrounds is essential along with a familiarity of the health disparities, challenges and areas of need among the Los Angeles community.

Essential Duties

  1. Enroll eligible patients into the HHP by performing outreach and follow-up with patients to maintain engagement in the program.
  2. Under the guidance of the Care Coordinator, assist with the development and maintenance of health action plans to help patients achieve their goals.
  3. Document patient interactions and activities in medical record as required.
  4. Accompany HHP patients to office visits, if necessary.
  5. Provide health promotion and self-management training to patients.
  6. Arrange transportation for patients.
  7. Distribute health promotion materials in culturally and linguistically appropriate formats.
  8. Call patients to facilitate HHP visits with Care Coordinator.
  9. Assist in the follow-up of specialty care referrals.
  10. Collect and track data on outreach efforts, submit monthly reports.
  11. Connect HHP patients to social services and supports he/she may need.
  12. Advocate on behalf of patients with their care team.
  13. Use motivational interviewing and trauma‐informed care practices.
  14. Monitor treatment adherence (including medication).
  15. Connect with unreachable patients by performing home visits, as needed.
  16. Work closely with housing navigator to coordinate home visits, as needed.
  17. Communicate to patients the purposes of the HHP and the impact it may have on their wellbeing.
  18. Help patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals.
  19. Assist in maintaining an accurate community resource database for organizational use.
  20. Travel to community locations, various agencies, and other outreach destinations as needed to meet with patients.
  21. Perform other duties as assigned.

Position Requirements

  1. 1. High school diploma or GED required.
  2. 2. English and Spanish fluency required.
  3. 3. Valid CA driver’s license.
  4. 4. Basic knowledge of computer skills.
  5. 5. Experience working in health care or related field preferred.
  6. 6. Experience working with patients or community members in the safety net environment.
  7. 7. Must be able to work in interdisciplinary team setting.
  8. 8. Effective communication and interpersonal skills.
